Library card applications

The last few library card applications I entered did not have any indication at the bottom of 1. type of patron, 2. what verification was checked (i.e., MxCC, driver's license, etc) and 3. initials of who accepted the application.  Just a reminder that this section must be checked.  Thank you, JH 2/2/12
